What Is a Bunion?

A bunion (hallux valgus) is a progressive deformity of the big toe joint. The big toe drifts toward the second toe while the metatarsal bone beneath it pushes outward, creating the characteristic bony prominence on the inner side of the foot. It is one of the most common foot conditions, affecting approximately 23% of adults.

Causes

Bunions have a strong genetic component, if a parent has bunions, you are significantly more likely to develop one. Wearing narrow, pointed footwear can accelerate progression but does not cause bunions. Associated conditions include flat feet, ligamentous laxity, and inflammatory arthritis.

Symptoms

• A bony prominence on the inner side of the big toe joint
• Big toe angled toward the second toe
• Pain, redness and swelling over the bunion — particularly with footwear
• Difficulty fitting shoes
• Progression over time, potentially causing lesser toe deformities (hammer toes)

Non-Surgical Management

Conservative measures can manage symptoms but cannot reverse the deformity: wide-fitting shoes, bunion spacers, orthotics to offload the joint, and padding over the prominence. Surgery is considered when pain significantly limits footwear choices or daily activity, and conservative measures have failed.

Minimally Invasive (Keyhole) Bunion Surgery

Dr Smith performs the Minimally Invasive Chevron Akin (MICA) technique, a modern keyhole approach to bunion correction. Through two or three 3–5mm incisions, specialised instruments are used to precisely cut and reposition the metatarsal bone and correct the big toe alignment under X-ray guidance.

Compared to traditional open surgery, MICA offers: smaller scars, less soft tissue disruption, earlier weight-bearing (in a surgical shoe from day one), and faster return to normal footwear (6–8 weeks).

Recovery

Patients walk in a flat surgical shoe from day one. Normal footwear is typically resumed at 6–8 weeks. Return to exercise (cycling, swimming) is possible at 6–8 weeks; running at 3–4 months. Swelling may persist for 3–6 months. Patient satisfaction with MICA is consistently high.
